Option B is incorrect because it only suggests that information 'should' be furnished. And it goes against the sentence meaning.

Why is A the only correct answer? D and E also seem to be correct.

D isn't correct because it completely changes the meaning of the original sentence from a suggestion to a "you must". I think the error in E is the "should have to" could have been simplified to just "should"
